Periodicals - Members of the CMS are eligible for significant reductions in the regular rates of all CMS publications. Please see our catalogue for a complete list of publications.
Please note that all information provided in the shaded fields of the database information area of the form is published in the CMS Membership Directory on the Website. Any member who wishes to be excluded from the membership directory must indicate this on the form.
Please complete the form, including all applicable shipping & handling charges and taxes, and send it to the appropriate address. Payment may be made by cheque, money order or by VISA or MasterCard. The CMS does not accept wire transfers.
For memberships, subscription publications, back issues and educational publications, those with Canadian mailing addresses must remit in Canadian dollars and those with non-Canadian addresses must remit in US dollars. For clients using VISA or MasterCard, a Canadian-US calculation will be done based on the current exchange rate.
When paying by credit card, please make sure to include the cardholder name, card number and expiration date and the cardholder signature.
SHIPPING AND HANDLING CHARGES
For subscription publications, shipping and handling charges (surface mail) are included in the rates. For all other publications, including back issues, shipping and handling charges are not included and should be added on the form. Charges for other postage options, where available, must be included on the form.
TAXES - GST/HST
The Canadian government imposes a Goods and Services Tax (GST) or a Harmonized Sales Tax (HST) on books and subscriptions sold to clients with Canadian shipping addresses in the following provinces and territories:
MEMBERSHIPS AND SUBSCRIPTIONS INFORMATION
Memberships and subscriptions are sold on a calendar-year basis (January 1 to December 31). Advance memberships and subscriptions may be purchased for a maximum of three years. Split-year membership and subscription orders are not available. Subscriptions are sold at back-issue prices after December 31 of the subscription year. Surface postage is included in the subscription cost. To defray higher postage and handling costs for subscribers outside of Canada, subscribers with non-Canadian mailing addresses must remit in US funds.
Individual and school on-line access is by means of a user ID # and password. Institutional on-line access is by means of a single internet domain and the price is based upon the domain serving a single campus.
MEMBERSHIP AND SUBSCRIPTION CANCELLATION
Cancellations for membership or subscription publications are accepted before March 1 of the current year. Refunds will be issued, less a $50.00 administrative charge per cancellation, and the cost of any issues already shipped, or pro-rated for online-only subscriptions.
MEMBERSHIP AND SUBSCRIPTION CHANGES OR UPGRADES
Changes or upgrades for membership or subscription publications are accepted before March 1 of the subscription year and are subject to a $25.00 administrative charge.
CLAIMS FOR MISSING JOURNALS, SHORT SHIPMENTS, DAMAGED COPIES, INCORRECT TITLES,
NON-RECEIPT OF BOOKS
Claims for missing journals may be made not before three months and not after six months of the date of the issue. Claims for short shipments, damaged copies, incorrect titles, non-receipt of books should be placed within three months of the date of payment.
Book returns must be authorised in advance and must be made within one year of the shipping date. Books returned to the CMS must be in resaleable condition in order for a credit to be issued. Once an authorised return is received, a refund will be issued, less a $10.00 service charge.
All CMS promotional items are non-refundable.
